Youth Volunteer Forms

Thank you for volunteering with St. Augustine & Joseph Catholic Church!

Please read the documents linked below, and contact Will Hahn to  receive volunteer packet and forms that are required to be signed.

After completing the forms, please turn in signed forms to the parish office! Please note that all items must be completed before engaging in youth activities with the youth of our parish in order to protect you and our youth!

-Ethics and Conduct Statement


-Ethics and Conduct Agreement


-Background Check This background check costs $25, a token to void the cost is attached to the packet you will receive from Will Hahn.
